We are a reception-less acupuncture clinic so please complete your new patient paperwork and bring to your first appointment.  Paperwork must be completed prior to arrival. 

Payment for services are due in full at the time of your visit. All forms of payment are accepted (cash, check, credit card and flex spending cards (HSA and FSA). At this time, I am  an in network provider with CDPHP, United Healthcare (most plans) and NYSHIP New York State Health Insurance Plan (The Empire Plan). However, it would be best to check with your insurance provider to verify acupuncture benefits for your specific policy prior to your appointment.  If your insurance plan does cover out of network acupuncture, we will gladly provide you with an itemized receipt to submit to your insurance for reimbursement upon request.

Please arrive to your appointment on time and wear or bring comfortable and loose fitting clothing for your visit.  It is also advisable to have a light snack before your appointment.
